Our Services

We aim to find the ideal care solution for you, offering from as little as 30 minutes to 24 hours of support per day. You can remain in the comfort of your own home while receiving the necessary care from fully trained healthcare professionals.

Our Care at Home services include;

  • Help with personal care

  • Assistance at mealtimes

  • Support with everyday tasks: cleaning, shopping and laundry

  • Overnight support

  • Companionship on holidays, trips or transfers

Service Users

  • In responding to the needs of Service Users, JACS Care will ensure fair access to its services. JACS Care will make sure that everything it does is transparent, regularly reviewed and updated in order to meet the changing needs of current and future Service Users.

  • Information, with staff support as necessary, will be made available to help to explain the Equal Opportunity Policy and any other key policy or procedure. This information shall be clear, concise and 'fit for purpose'.

  • We will take into consideration any communication or language needs of clients, Service Users, and other stakeholders in all future service planning and development.

Assessment of clients will include;

  • Risk from the living space or accommodation

  • Personal care needs

  • Mental health needs

  • Mental capacity status

  • Skin marks

  • Skin integrity

  • General

  • Moving and handling

  • Nutrition

  • Falls

  • Medication

  • PRN medication

  • Social care

  • Any other risks specific to the Service User.

  • JACS Care's complaints policy will be made available to all Service Users, parents/carers, advocates, friends and befrienders and the public as we welcome complaints in order to maintain our standards and to improve service delivery.

  • Service Users will be encouraged by staff at JACS Care to self-advocate by making a complaint themselves if required or will be supported to access Advocacy Services to do so.

  • Service Users are asked to respect the rights of others. There is a Safeguarding Policy in place to protect them from abuse.

Care Plan

Service Users will have the Care Planning purpose and processes explained to them and be informed that they have the right to ask for a Care Plan review meeting at any time.

Review

Care Plan reviews should assess the action records generated by staff in order to judge the success of the Care Plan in achievement of the planned outcomes, and identify changes to the Care Plan which are required to meet existing, changed or new needs.
